This form should be included with any items shipped back to CTS for repair. You will need to obtain an RMA# from a customer service representative by calling 800-279-0111 x 3 prior to shipping. Any other possible resolutions will also be discussed at that time.
Should you require on-site assistance with your products, you will need to request satellite services by filling out this form.
This form must be included with all touchpad orders.
