This form is available for all customers to request support for our products. Before submitting, please review the following forms that may need to be included with your submission.
This form should be included with any items shipped back to CTS for repair. You will also need to obtain an RMA# from a customer service representative prior to shipping.
Should you require on-site assistance with your products, you will need to request satellite services by filling out this form.
This form must be included with all touchpad orders.
